I have the potential of working as an advanced nurse not only based on my background education in nursing but because of my personal experiences of how nurses can help patients and their families during sickness. My father was diagnosed with brain cancer and this was a difficult time in our lives. Medical Testing on Animals Essay Animal experiments are widely used to develop new medicines and to test the safety of other products. Many of these experiments cause pain to the animals involved or reduce their quality of life in other ways. If it is morally wrong to cause animals to suffer, then experimenting on animals produces serious moral problems. Animal experimenters are very aware of this ethical problem and acknowledge that experiments should be made as humane as possible. It is also known that it is wrong to use animals if alternative testing methods would produce equally valid results. Experimenting on animals is always unacceptable because it causes suffering to animals, and the benefits to human beings are not proven. The case against animal testing is that the level of suffering and the number of animals involved are both so high that the benefits to humanity do not provide moral justification. The equivalent case for animal experiments is that they will produce such great benefits for humanity that it is morally acceptable to harm a few animals. Animal experiments are not used to show that drugs are safe and effective in human beings, as they cannot do that. Instead, they are used to help decide whether a particular drug should be tested on people. Animal experiments eliminate some potential drugs as either ineffective or too dangerous to use on human beings. If a drug passes the animal test it is then tested on a small group of humans before larger trials are done. Animal experiments only benefit human beings if their results are valid and can be applied to human beings. Another problem with animal testing is that results can be misleading. Drugs that have negative effects on animals could potentially be highly beneficial to humans and vice versa. Those in favor of animal experiments say that the benefits to human beings outweigh the harm done to the animals being tested. This is a consequentialist argument, because it looks at the consequences of the actions under consideration. It cannot be used to defend all forms of experimentation since there are some forms of suffering that are probably impossible to justify even if the benefits are exceptionally valuable to humanity. The consequentialist justification of animal experiments is done by comparing the harm the experiment will do to animals to the harm done to humans by not doing the experiment. This is not a valid way to justify the argument because this is ultimately comparing two different things. The harm that will be done to he animals is certain to happen if the experiment is carried out. The harm done to human beings by not doing the experiment is unknown because no one knows how likely the experiment is to succeed or what benefits it might produce. It is morally worse to do harm by doing something than to do harm by not doing something. Looking at it that way, it is morally worse for experimenters to harm the animals by experimenting on them than it is to harm some human beings by not doing an experiment that might find a cure for their disease. The benefits to humans does not outweigh the pain and suffering that all of the animals endure while going through experiments. The issue of animal experiments is very clear if we accept that animals have rights. If an experiment violates the rights of an animal, then it is morally wrong because it is wrong to violate rights. The possible benefits to humanity by performing these experiments are completely irrelevant to the morality of the case, because rights should never be violated.

Individual Patient Care in Dementia A nurse’s role focuses on the help, care and support given to their patients whilst treating people as individuals and upholding their dignity (The NMC code, 2015). In this essay I will discuss the ways in which nurses can ensure that patients with dementia receive individualised patient care. The aim of this essay is to demonstrate how care is implemented to patients with dementia and how nurses ensure care is individualised to meet the patients’ needs and wants. Nurses can identify the individual needs of the patient by following the nursing process.   The nursing process is a series of stages intended for nurses to demonstrate excellent care. It consists of five phases: Assessing, diagnosing, planning, implementing and evaluating.   This process is client centred. These stages mean that nurses should individualise what is needed for one patient.   A patient needs, and problems is identified through these steps. The Assessment phase is the first step in which it allows nurses to identify what the patient’s needs are. The nurse collects information from the patient by asking them questions and running physical examinations. They dissect the information that is gathered in this stage in which it is further analysed which requires in dept thinking. The Diagnosing Phase is the next phase in which the nurses make an overall diagnosis about the information that was collected in the assessment phase (Gardner, 2003). It is stated that patients tackle a medical diagnosis with what mental health professionals name an anticipatory anxiety. They are nervous and scared as to what they are told and how it may change their day to day life for them and their close ones (McClain and Buchman, 2011).   The diagnosis of dementia entails of examination, cognitive testing and assessment. Nurses informing patients that their memory and cognitive function is beginning to change can be challenging and difficult to hear hence it is crucial that nurses should uphold their dignity and inform them of the treatment that will be applied and to give them the help and support they need. (Prince and Martin, 2016). The planning phase lets the nurses create a plan of action in which ongoing treatment will be discussed. This phase allows the nurses to address patient’s needs. The implementing phase is when nurses carry out the plan of action. For dementia patients their symptoms tend to go worse. It is vital that nurses demonstrate great care in which they can them support with daily activities e.g. washing and dressing them. They should also monitor the patient and focus on the improvements made by the patients. It is vital that the nurses care, monitor and support that is given to the patients is continuous. The care that is received by the patients with dementia is much lower as to those patients without dementia hence it is fundamental that the nurses ensure that care is individualised to the patients’ needs and wants.   For the last evaluation phase, it is crucial that nurses complete an evaluation to see if the treatment that was carried out is working and if any changes happen. If the treatment isn’t working nurses can support the client, analyse and understand as to why it didn’t work (Gardner, 2003). Nurses should respect the patient’s beliefs and prevent making assumptions mainly grounded on their appearance or other personal quality. They must listen and consider patient concerns. It is vital that the nurse is non-judgmental and open minded towards the patient. Nurses can ensure care is individualised when it comes to fulfilling the nutrition, pain management and personal needs of the patient. If the patient cannot manage or is unable to regulate their nutrition, then the nurse should support and encourage the patient by placing food within their reach (Kaplan, 1996). Providing care to a patient who suffers from dementia is vital as the patient does not have the ability fully understand their diagnosis. As a nurse, it is encouraged to introduce yourself to the patient to create a therapeutic relationship during treatment. Patients who have dementia are no longer able to maintain their individuality and personhood hence why it is important that nurses can try and uphold and preserve it for them. Patients value nurses recognizing their individuality. Nurses reassure patients that one is not living a horrible and unhappy life by implementing the worth and value to their life by trying to get to know the person behind the patient. Nurses can ensure that care is individualised as they could get to know the individual, their values, likes and dislikes and hobbies as this gives the patient an individuality whilst always showing compassion and respect (Collins and Hughes, 2014). This is most valued and appreciated by patients as it allows the nurses to know the characteristic and the personality of the patient.   Nurses can show recognition to the patient by acknowledging their needs and wants and providing care that is customized and adapted to it. It is important that nurses try and build an insight of the patient’s world and how to bond with them. When communicating and engaging with them they must always say their name unless the patient wishes a different way of being addressed.   Nurses can consider the patients perspective when demonstrating care that is exclusively personalised to their needs.   Giving recognition to the patient allows the relationship to build much stronger as you are giving your attention and time to them.   Nurses would give the patients the choice and responsibility to make their own decisions when it comes to their choice of food, clothes they want to wear, getting involved in activities etc. Allowing the patients to make decision like this lets them know that they are comfortable. It also gives them a sense of involvement and participation to express their qualities and personality. However, when the discussion of making clinical discussions arises and the patient is unable to make the decisions due to cognitive abilities declining, the family and doctors will be more involved. Nurses should allow the patients to create their own pace in which you shouldn’t push the patients over their limits. It would be much of a benefit to focus on the improvements made by the patients even if it’s something small. This would motivate and drive the patient building their self-esteem. When a nurse is caring for a patient who has dementia it is important that you do not patronise them. Respect for the patient is a main aspect nurses must implement in their duty of care. Nurses can ensure that the care and treatment given to the patients is with both respect and compassion (The NMC code, 2015). Socialisation and interaction is fundamental for patients as it allows the patients to maintain a social life and form relationships. Allowing the patients to experience and be around company will progress their communication skills. Nurses should recognise that all patients including people with dementia is built in relationships and that dementia patients require a healthy social environment to promote opportunities for personal and mental progress.   Dementia affects the way a patient communicates. People suffering from dementia can find difficulties responding back to question (NICE, 2012).   When conversing with patient with dementia they may also find problems to maintain the information during a discussion.  Nurses must validate and shouldn’t dismiss what is said by the patient. They must try to understand and take notice of what the patient expresses to them.   Nurses can communicate in a calm and respectful way in which they should speak directly to the patient. It can be frustrating for a patient with dementia to communicate their needs and wants hence it is vital that nurses are supposed to remain calm and patient if the patient becomes agitated (Ellis and Astell, 2017).   When a patient’s conditions begin to deteriorate, health and social care needs begin to increase causing them to require more help and personal care. When nurses are relaying information to a patient they should give the patient both oral and written information, so it can be fully understood and so it can encourage and boost their communication skills in their care and treatment. When released from hospital, people with dementia are likely to suffer a serious loss of individuality, and increased needs for help and support. So, it is important that the care is demonstrated to patients not only during hospital but when they arrive home.   Nurses can try and view the world from the viewpoint of the person with dementia, distinguishing that everyone’s experience has its own psychological validity, that people with dementia act from this outlook (Brooker, 2007). The ageing population is exponentially increasing resulting in challenges to nurses in coping and treating the conditions and health needs that arise with old age (Bhardwa, 2015). These barriers that I will be explaining are obstacles that prevent the pace of excellent care being demonstrated by nurses. The barriers to demonstrating care to dementia patients is that they receive poor quality in which nurses tend to focus on other patients with severe illness and diseases.   Another barrier would be ineffective advance care planning. Some people with dementia receive a delayed diagnosis which can result in them not having the mental capacity to attain decisions. A lot of patients find it difficult to vision their self-getting better due to their current state. Nurses can ensure that the care that is provided to people with dementia is quality care during the duration of their treatment. Hospice use is incredibly low for dementia patients. The people with dementia that get transferred to a hospice can result in confusion and distress at a state in which the person is unable to handle change. Also, they have completely different needs compared to cancer patience’s. It is crucial that both staff and nurses have the training required to deliver care to individuals with dementia. Age discrimination is also a barrier that elderly patients face in which the symptoms demonstrated to doctors and nurses is referred to a getting old. Nurses are failing to spot and notice the symptoms of dementia in a lot of patients which creates a poor rate of diagnosis (Collins and Hughes, 2014). The organization like the National Health Service also create barriers resulting in patients not receiving the care they need. They have limited access to resources, lack of time, heavy patient workloads and insufficient staffing. Nurses have a contribution when it comes to the barriers of providing care to patients. Some nurses have a lack of interest, lack of confidence in critical appraisal skills, lack of knowledge and them feeling overwhelmed (CAN, 2018). Dementia patients experience behavioural and personality changes. Patients that specifically have advances dementia tend to be physically aggressive, have hallucination and get agitated. These symptoms can result physical and emotional distress to both the patient and the nurse. There is also hostile treatment for dementia patients that is very familiar in which it consists of tube feeding and antibiotic treatment for infections. This treatment is known to be wrong and does not improve survival. Families of the patient shows great dissatisfaction against the aggressive treatment that is demonstrated to the patients. Nurses can implement excellent care by concentrating on improving patients comfort and increase in advance care planning (Collins and Hughes, 2014). The points I explained in this essay show how providing and offering care to people with dementia can be complex and there can be a lot of boundaries that come along with it however when the when the needs, wants, choices and problems is focused and centred around the patient that’s when care is at its best. Injuring his back even more, Kennedy still managed to pull another injured crew member 3 miles across the ocean. He reached an island where he and his crew were eventually rescued. He received numerous medals for his courageous act. Kennedys older bother, Joseph Jr., was killed in WWII. To try and fill his spot, John F. Kennedy became active in politics. In 1946 a seat in the House of Representatives opened, Kennedy ran for the seat and beat his opponent. He was reelected twice, but there was a mixed voting record. Then in 1952 he ran for Senate using the slogan â€Å"Kennedy will do more for Massachusetts†. Kennedy was considered the â€Å"underdog† by most, but he still came through and defeated Henry Cabot Lodge Jr. with a margin of approximately 70,000 votes. In 1951 John F. Kennedy was considered â€Å"the most eligible bachelor†. At a dinner party in May he met a beautiful young woman named Jacqueline (Jackie)Bouvier who interviewed him for a newspaper column. There paths crossed once more, but this time it was different. She had gotten engaged to John Husted in December of 1951. But they were interested in each other , so Jackie broke off her engagement in March of 1952. John introduced her to his family that coming summer. They dated very privately for a while. Johns father was I impressed. He thought she had a enormous amount of class, which was a characteristic he found suited for the wife of a politician. About a year later their engagement was announced on June 24, 1953. They had a short engagement. They were married on Sept 12, 1953, at St. Mary’s Roman Catholic Church in Newport, Rhode Island. Unfortunately he was not chosen. This didn’t make him any less ambitious. He decided to run for president in the elections of 1960. Kennedy won the primary elections, despite suspicions about Kennedy being Catholic. Then in July of 1960 the Democratic Party elected Kennedy to run for president. He then asked one of his opponents in the primary elections, Lyndon Johnson, to be his running mate. He needed Johnson for the southern states. This election was already predicted to be a close one. His opponent was Vice President Richard Nixon. Kennedy and Nixon debated in September and October in the very first televised US presidential debates. Having the presidential debates televised was monumental. It created a whole new aspect in deciding who would win. On TV Kennedy was way more calm and composed, where Nixon seemed nervous and unsure. But then on the radio, it was Nixon who sounded like the more pronounced speaker. On November 8 Kennedy beat Nixon in a very controversial race. Even though Nixon had won the Popular votes, Kennedy still won the Electoral votes. On January 20, 1961 Kennedy was inaugurated. Kennedy made a step in the right direction when he created the â€Å"Peace Corps†. This program was made to aid underdeveloped countries. Kennedy dealt with many things during his presidency They In April of 1960, Kennedy gave orders to invade Cuba. This act is known as the â€Å"Bay of Pigs Invasion†. They were hoping to take Fidel Castro out of power. Within a couple of days, Castro’s government had killed most of the men, and he had to negotiate to get the rest home safely. When the U.S. obtained photos showing that Soviets in Cuba had ballistic missiles sites in construction, Kennedy was in a tough positions. If he tried invading, that would put the U.S. in great danger. But if he did nothing the U.S. would still be in danger and the U.S. could seem weak. About a week later, Kennedy started negotiating with Soviet leader Nikita Khrushchev. Luckily they came to an agreement and the missiles were removed. Needless t o say, Kennedy was careful when confronting the Soviet Union. That was only foreign policies! Here in the U.S., Kennedy dealt with the Civil Rights Movement. Even though the Supreme Court had ended segregation in schools, there was still schools not abiding by the laws. Segregation in local businesses, and on buses were becoming heated issues. Kennedy sent federal marshals and troops to ensure that a student was able to enroll in a particular college. Kennedy supported racial integration and civil rights. In 1963 Kennedy he also made one of the biggest tax cuts in U.S. history. Larger then the one made by Reagan. Kennedy supported the â€Å"Space Race† in which the U.S. competed against the Soviet Union on exploring space and landing a man on the moon. The U.S. was behind in this race, and he wanted greatly to catch up. He asked congress to approve more then twenty two billion dollars for â€Å"Project Apollo†. Its goal was to have a man on the moon by the end of the decade. Kennedy’s presidency had many successes. But on November 22, 1963 his success would come to a devastating end. When Kennedy arrived in Dallas, Texas he stepped of the plane flashing his golden smile for the tons of people who were there to get just a glimpse of him. He was in Dallas to make some speeches. The day was like any other day for the president. He was riding in a car, top down, sitting beside the first lady. Their was a massive crowd gathered on each side of the street, watching as the president passed, waving. Then all of a sudden, through all the excitement, a shot was heard. The president fell over onto his wife. There was chaos amongst the all of the vehicles in the line. Then another shot was fired and that bullet hit Senator Connally. One more shot was fired, hitting Kennedy in the back of the head. The two cars sped to a nearby hospital. The doctors did all they could, but John F. Kennedy died at approximately 1:00 p.m. on November 22, 1963. His death traveled fast by phone and the media. They should act upon the publicaâ‚ ¬Ã¢â€ž ¢s best interests, and not single out any party or group whose stance on a matter should be determined as any less significant. If a decision is detrimental to anyoneaâ‚ ¬Ã¢â€ž ¢s life in the general public, then it is likely there will be more than one person affected by the same things. The government should act upon their doings with precision and a conscious understanding of their impact on everyone. Dependant on the necessity of their actions an act should not be passed if it is going to exclude even a minority in the populations opinions. With this we can determine clear goal sets and be unified. When going through with anything you should always check your tracks to see if you stomped on any bugs. Through out the Vietnam War these ingredients were missing for the American position. It doesnaâ‚ ¬Ã¢â€ž ¢t matter if the opponent is right or wrong, but if you are following these tactics, you will be successful. Ho Chi Minh and his communist following was ensuing power through authority and fear. Due to the fear he was possessing in many of the Vietnamese civilians, he was able to create unity for the goal sets he was trying to reach. His methods consisted of using fear over his people to squeeze out the product he desired. Once the people saw the product he achieved by communist ideals, Minh created a massive following of communist believers to ensure the continuation of reaching their goals and his regime. He established groups such as the NLF to spread his ideals and expand his following, but also to support all sections of his military. They believed that aâ‚ ¬? as long as they did not lose, they won. aâ‚ ¬? Americans on the other hand should have seen their defeat coming. Constantly reflecting on the Korean War as aâ‚ ¬? learning our lesson,aâ‚ ¬? we should have taken the French defeat in Dien Bien Phu as a warning. Americans entered the war, head first with no structure of unity, goal sets, or ideals. Events such as the Tet Offensive had not only initiated a new phase of the war but also showed us that our unity was weak. The US, if unified would have been smarter to not abandon all their cities and create opportunity for the NLF and Vietcong to strike. We would have communicated and sorted our priorities to know our goals before separating into remote areas. Other battles like Ia Drang showed the issues with unity and goals that have become noticed because of the failure to succeed in communication during them. The Pentagon Papers along with some accumulating underground news articles revealed much of what went wrong to the public in 1971. When the Americans had already initiated Vietnamization it was to late to change our strategy for the war effort. The papers caused discourse within our own country and distrust of our government. They were one of the large breaking points to all the built up distress over the war. In the papers we lied about the Gulf of Tokin aâ‚ ¬? attackaâ‚ ¬? and covered up our faults to our own people. Events such as My Lai concerning Lieutenant William Calley posed even more issues on the moral justification we had in Vietnam, for such things as our aâ‚ ¬? Free Fire Policyaâ‚ ¬? or aâ‚ ¬? Search and Destroy Campaign. aâ‚ ¬? It showed that Americans went against their own ideals when in Vietnam by not treating the Vietnamese with equal liberties, as we would give ourselves. It seemed as though we held a higher standing for ourselves when in Vietnam so we did not follow our own belief of treating all humans as equals. Killing innocent civilians without trial, not accepting the cultural difference of nations, using the threat of arms as a bribe to follow our beliefs, treating foreigners with out equality, ect. It was all that went wrong in our approach to try and gain the Vietnamese trust. When we went over to Vietnam we showed, a third world country in despair, a country that does not constitute itself by the ideals we arrived there with. In fact we did the exact things we were fighting against and did what we originally founded ourselves to not become, in Vietnam. It was a hypocritical scenario where we couldnaâ‚ ¬Ã¢â€ž ¢t learn from our mistakes until we were gone and had already lost. In present day Afghan/ Iraq with many of those mistakes from Vietnam still hanging over our heads, we are now pushing to fix. Since Vietnam we have initiated the Special Operations Unit, and founded many other sections of the Military to ensure we donaâ‚ ¬Ã¢â€ž ¢t make those same mistakes. Although many things are still missing from our strategy, such as going against our ideals again in the case of Abu Ghraib, we have come a long way. A friend of mine, Richard Bennett, currently serving in the US Special Opps. Group, and waiting to be deported to Afghan/ Iraq is anxious to utilize his training. His job will be to train civilians on how to protect and sustain their own government, so they can stand on their own. He has gone through rigorous training to ensure his capability in every situation but as his mother said to me in her interview, aâ‚ ¬? Richie is anxious to be deported and utilize his trainingaâ‚ ¬Ã‚ ¦ but he has expressed his frustrations with the politics concerning the waraâ‚ ¬Ã‚ ¦ like I said, it is the Age of Information, so you can see multiple sides to every story now. aâ‚ ¬? Essentially her point was to express that even if there is corruption in Afghan/ Iraq there is practically nothing the public cant obtain information on. The publicaâ‚ ¬Ã¢â€ž ¢s judgment on the issues should be heard and taken into consideration by the government to change what they feel is not right based on our ideals. Finally, when analyzing the trends and mistakes we made in Vietnam there is significant improvement of our structure in Afghanistan/ Iraq now. Even though we still have our defects as citizens, we should always remember the three components to being successful as a country and as a citizen of that country. The unity, goals, and ideals of a country found their infrastructure. So to be a good citizen you have to work to maintain that, for your country, and for the future of your country.
